Diesel-hauled passenger trains are almost gone by now.
Some coach-based trains powered by SU42 and SP32 diesels can be often found in the West (Krzyż, Chojnice, Piła region) to strenghten the railbuses. Usually 2 coaches of 111A or 120A type powered by SP32 or SU42 diesel. Ocasionally one of last operational SU45s is used for that kind of work.
A long distance train from Zamość to Kraków/Wrocław is lead by an SU45 between Zamość and Stalowa Wola, it's a PKP Cargo locomotive, rended for IC service. Usually 3-4 coaches.
The last bastion of diesel passenger work is the norht-eastern region. Two non-electrified lines keep some diesels alive - Ełk-Korsze trail and Białystok-Suwałki. Generally 4 SU45s are used there, but their condition is very poor, they are often broke down and another locomotive is used - generally SM42 or ST44s, SU46s rented from Cargo.
One EC train is an exception. EC 'Wawel' train from Kraków to Hamburg is operated by cargo-rented SU46 class between Legnica and Forst.
And that would be all... maybe except for summer seasonal trains around Gdynia. Usually consisted of SU42s or SP32s with random coaches, including famous double-deck sets (2 of them still operational summer 2012).

According to this answer I would add that one of EC train from Gdynia to Berlin would be operated by SU46 nearby Poland - Germany border (the same renting rules), but I'm not sure.
And at Masuria region (for ex. Korsze - Ełk) last long-distance trains are dominated by SU45 (TLK54108 POJEZIERZE, TLK18105 RYBAK) or SM42
